What is a care manager? 

There are many components to Medicaid-based home care. Once you’ve gone through the assessment process and you have qualified, you’ve probably enrolled in an MLTC (Managed…) insurance plan. The plan will have a nurse conduct an assessment (known as a UAS), and ultimately approve you for hours, based on your specific needs. 

The next step is, that your Medicaid-funded insurance plan will assign you with a care team, led by a care manager, who will oversee your home care. 

While it may sound simple, the role of a care manager is intricate and involves many aspects.
