At Harbor Care, we encourage our patients to enjoy the freedom and independence of choosing your own caregiver through the Consumer Directed Personal Assistance Program, or CDPAP.
To qualify for CDPAP, consumers must need assistance with day to day activities or skilled care. They must be self-directing, or otherwise have a designated representative who can and will direct care under the CDPAP requirements.
Harbor Care is licensed by the New York State Department of Health and Accredited by the Joint Commission (JCAHO). We train, screen and supervise our caregivers. We offer a full range of services.
Under New York’s CDPAP program, patients can take ownership of their home health care and select a caregiver that they know and trust. Rather than bringing in a stranger to care for your loved one, you and your family can have the final say in who is providing the home care services they need.
Any patient who qualifies for Medicaid and requires home care, personal care or skilled nursing services, can sign up for the Consumer Directed Personal Assistance Program (CDPAP).

No. The caregiver or the representative will not need any kind of formal training, license or certificate to be employed. Any skills that might be needed for this opportunity will be taught directly by the patient.
Harbor Care offers competitive rates and benefits to all CDPAP caregivers. We process payroll every week so that a weekly check will be distributed.

No, the patient assistance is provided by the consumer/patient or the designated representative. In the event that a patient assistant cannot continue to work, the consumer or the designated representative is responsible for restaffing the patient assistant.
